ADOPTION/HEALTH CONTRACT + PET ONLY UNLESS OTHERWISE STATED AT THE BOTTOM OF THIS CONTRACT
I, the buyer acknowledge that I have received a copy of the Florida pet lemon law. I understand that by signing this contract that I am waiving my rights for refund or exchange for hereditary or congenital diseases. I agree and will abide by the sellers contract and guarantee.
All puppies are sold as pets only, but the buyer will be given full AKC registration rights.
All of our puppies will come with current shots for their appropiate age. Your puppy has been dewormed for the prevention of worms. Your puppy has a State of Florida health certificate issued by a licensed Veterinarian and has been found to be in good health.
Our contract is as follows: UPON OR BEFORE ADOPTION THIS CONTRACT MUST BE SIGNED AND UNDERSTOOD.
We guarantee our puppies against contagious and infectious diseases for 72 hours. In the event of a return, the puppy must be diagnosed clinically ill by a licensed Veterinarian. Seller has the right to receive copies of all medical documentation concerning the puppy. Seller has the right to verify this by his or her own veterinarian. The puppy must be returned to the seller. If this is an infectious disease do to the risk to other puppies we will not take the puppy back, but we will refund your full purchase price. xx________
If you do not seek a veterinarian within 72 hours (excluding Holidays and weekends) warranty is void, and breeder is to assume puppy is in excellent condition.xx_________
If deceased, you must provide a statement from a licensed Veterinarian as to the cause of death. If found that the puppy was sick at time of purchase, a refund for full purchase price will be given. (I understand that I (the buyer) am responsible for the return of the puppy and any costs involved).
AKC now offers a free 60 day health plan when you register your puppy. This is at no cost to you for 60 days. It is up to you to continue with this plan after this limited time.
We offer these extended warranties: Should your dog be found unfit for purchase within one year from the puppies date of birth and this disorder adversely affects the animals health, this being due to a congenital or
hereditary disorder,the following will apply:
HIPS- We offer a twelve month guarantee that pertains to only moderate or severe hip dysplasia. The opinion or rating must be documented by the Orthopedic Foundation of America with an official OFA or Prelim. and no other Doctor or medical institution.
If your dogs hips are moderate or severe you can do the following:
1. RETURN THE PUPPY AND RECEIVE A REPLACEMENT PUPPY (of equal value from a litter of sellers choice)
2. RETURN THE PUPPY AND RECEIVE A FULL REFUND OF PURCHASE PRICE.
3. KEEP THE PUPPY AND RESUME ALL RESPONSIBILITY FOR HIS OR HER WELL BEING.
We have the right to contest any refund or exchange. We have the right to require our own veterinarian to examine the dog and come to his own conclusions, as to the nature of the problem. We do offer extended
warranties beyond what is stated on this contract Doctor or medical institution.

WE DO NOT INCLUDE THE FOLLOWING IN OUR GUARANTEE:
Demodectic, skin problems or allergies. Eutropia eye lid problems. We do not offer returns or exchanges or money refunded for anything of or related to the reproductive system. Such as testicles not dropping or being unable to become fertile or produce. These problems do not pose a threat to a family pet. These are common and curable and can also be related to environmental causes. xx________
We do not offer any extended warranties for breeding or show. The same warranties apply. However, if you are given full registration and decide to breed or show your dog, we do not refund money on dogs that do not qualify for either. xx___________
We do not guarantee puppies or dogs for color for being lighter or darker than expected or smaller or larger than expected.
By signing this contract/agreement it is understood that as a consumer I waive all rights except as applies to seller's contract. I understand whoever signs the contract or picks up the puppy is responsible for the contents
regarding the sale of this puppy. This person is fully responsible for any shipping and transportation fees, if needed for a return. The buyer is assuming all responsibility for any veterinarian fees for exams, drugs, x-rays,
surgeries or any other treatments or options hereafter.xx_________
Breeder is selling this puppy outright to buyer. Breeder will provide buyer with a signed American Kennel Club application with NO limitations upon completion of sale, payment in full and contracts signed. Puppy purchase price is $_________.
ADDITIONAL TERMS AND CONDITIONS.
1. Under NO CIRCUMSTANCE is this puppy/dog to be placed in a pet shop, animal shelter or similar establishment, no kill or otherwise.
2. Under NO CIRCUMSTANCE is this puppy/dog to be used at any time in dog fighting, baiting, or any similar activities or illegal activities.
3. Under NO CIRCUMSTANCE is this puppy/dog to be knowingly sold to a laboratory, dog broker, or puppy mill.
